Health Insurance

Dr. Chollet is an in-network provider for the following health insurance companies:

 

• BlueCross BlueShield of VT

• BlueCross BlueShield Federal Employee Program

• Cigna

• MVP

• Harvard Pilgrim (some plans) HMO plans require a referral.

• Green Mountain Care ( VT Medicaid and Dr. Dynasaur)

• NOTE: Medicare does not cover our services.

 

We are happy to bill these insurance companies if they are your primary insurance; however, we cannot guarantee that our services will be covered. Even though Dr. Becky may be an in-network provider with your insurer, she may not be covered by your individual plan.

It is your responsibility to determine the extent of your coverage before your visit. We recommend that you call your health insurance company or HR/Benefits Coordinator to verify that Dr. Becky is an in-network provider for your plan, as well as to confirm that your plan covers naturopathic or alternative medical care. (Some plans exclude alternative medical care in the fine print.) To determine coverage, you may need to provide the health insurance representative with the following information:

3. Provider NPI Taxonomy: 175F00000X (naturopath)

4. Type of Visit: routine evaluation and management office visit or telemedicine visit

5. Service/CPT Code: New patient visit: 99202, 99203, 99204, or 99205
   Established patient visit: 99212, 99213, 99214 or 99215
   NOTE: The specific code cannot be determined until after the visit and is based on insurance mandated parameters that include consideration of total time spent on patient care (not just time spent face to face) and/or complexity of the condition.

We also recommend that you understand your co-pay, co-insurance and annual deductible obligations, as well as how much of your deductible you have already met before each visit.

Please notify us if you have a secondary insurance plan or other kind of health benefit like an HSA, HRA, or FSA.

Appointment Fees

​

Fees are determined by the length and complexity of the visit, but you can expect:

• First Office Visit: $170 - 310 (typically 1 hour, but we set aside extra time)

• Return Visit: $75 - 240 (typically 20-40 minutes)

For patients who do not have insurance or whose insurance doesn't cover us, we offer a discounted rate for paying in full at the time of the visit.

Good-Faith Estimate

Under the federal No Surprises Act, health care providers are required to give an estimate of the total cost of medical services to patients who self-pay in order to protect patients from unexpected medical bills. If you are uninsured, if you choose not to use your health insurance, or if Dr. Becky Chollet is out-of-network with your plan, you may request a written Good-Faith Estimate for any service you wish to schedule at our office.

Payments are due at the time of service

We require payment for insurance co-pays or appointment fees and any natural medicines dispensed at the time of service. We accept cash, checks, MasterCard, Visa, Discover, and American Express.
